Black ginger polymethoxyflavones deserve a close look. Unlike many supplement ingredients, they are supported by human randomized controlled trialsβnot just animal studiesβbut the measured effects are far more modest than many weight-loss ads suggest.
The most important conclusion is straightforward:
The most solid evidence currently shows that this effect is "a slight reduction in abdominal fat, especially visceral fat, after long-term consumption."
The clearest evidence is for 12 mg/day of black ginger-derived polymethoxyflavones (PMFs) for about 12 weeks. Two randomized, double-blind, placebo-controlled Japanese trials reported very similar reductions in visceral fat areaβabout 3.6 cm². This is not a powerful weight-loss ingredient and it does not burn hundreds of extra calories a day. It is better understood as a modest metabolic aid supported by repeat human evidence.
This article reviews research evidence and is not a substitute for diagnosis or treatment. Anyone who takes medication, has a chronic condition, or is pregnant or breastfeeding should consult a physician or pharmacist before using black ginger extract.
1. What exactly is it?
"Black ginger" is not the ordinary ginger used in everyday cooking.
Its botanical name is:

A very important point is:
PMF is not a single chemical but a group of structurally similar methoxyflavones.
In the 2021 clinical trial directly studying "purified black ginger PMF," HPLC confirmed that the standardized raw materials contained at least the following six main PMFs:

3. Reducing visceral fat: currently the most reliable effect
2018 Japan RCT
This is a very important study.
76 Japanese men and women aged 20~64, BMI:

The main endpoint is not the scale, but the CT scan:

4. The 2021 Trial That Isolated PMFs
This is the most valuable research item.
Because the problem in 2018 was:
Because the trial used whole black ginger extract, it could not show whether PMFs alone produced the effect.
So the researchers further purified PMF.
80 Japanese adults:

Randomly divided into two groups.
Each capsule contains PMF:
6 mg

Participants were also required to basically maintain their original lifestyle.

And the results of the entire black ginger extract in 2018 were:

The two figures almost coincide.
This result is quite interesting because:

Therefore, researchers believe:
The main active part of black ginger's ability to reduce visceral fat is likely this group of PMFs.
This is why Japanese products are often no longer simply written as follows:

Instead, he specifically wrote:

5. Is the effect big or small?
This place is very easily misled by advertising.
A reduction in visceral fat area of 3.5~3.7 cm² is a "measurable, statistically valid small effect," not a very obvious weight loss effect visible to the naked eye.
In other words:
After 12 weeks, you shouldn't expect the following:
"Your belly is clearly a bit smaller."
A more reasonable understanding is:
Without strict weight loss interventions, CT can detect a relatively small but repeatable change in abdominal fat.
Classification based on actual effect:
Does it work: Most likely, it does.
Strong effect: Not strong.
Is it a weight loss pill level: Absolutely not.
Is it more reliable than a large number of "fat-burning ingredients" with no evidence of human health? Yes.
6. Can it really "boost fat burning"?
There is some basis, but this is also where advertising is most easily exaggerated.
In 2015, a very interesting human experiment was conducted.
Twenty healthy men aged 21~29 were selected using a randomized crossover design.
Single intake:
100 mg of black ginger extract.
The main PMFs detected by HPLC from this extract include:
5,7-dimethoxyflavone: 4.07%
3,5,7,3β²,4β²-pentamethoxyflavone: 4.25%
3,5,7,4β²-tetramethoxyflavone: 2.16%.
Moreover, 6-gingerol and 6-shogaol, which are common in regular ginger, were not detected.
At 60 minutes of intake:
The change in energy consumption rate relative to baseline is approximately:
Black ginger: +229 kJ/day
Placebo:
β1 kJ/day.
Those with high BAT (brown adipose tissue) activity respond more noticeably, approximately attaining:
+351 kJ/day instantaneous energy consumption rate variation.
However, this should not be interpreted as:
"An additional 229 kJ of fuel per day."
Because this number was measured at a certain momentThe energy expenditure rate converted into a daily unit.
In fact, during the 90-minute experimental observation, the cumulative increase in energy expenditure in the black ginger group was only about:
10.4 kJ
While the placebo group was about:
β0.4 kJ.
So the "boost in metabolism" does have an experimental signal, but the extent is far from the exaggerated claims of a "fat-burning miracle."
7. Possible Mechanisms of Action
Current research has formed a relatively reasonable mechanistic chain:
PMF β Regulation of energy metabolism β Increased fat oxidation/lipolysis + Reduced lipid accumulation in fat cells β Slight long-term reduction in abdominal fat.
However, many links in the mechanism are still mainly derived from animal and cell experiments and cannot all be considered proven in humans.
1. Brown Adipose Tissue (BAT)
BAT is different from white fat, and one of its main functions is:
To convert chemical energy into heat.
In a 2015 human experiment, it was found that the effect of black ginger on increasing energy expenditure mainly appeared in:
People with higher BAT activity.
People with low BAT activity did not show obvious responses.
This means black ginger may not raise basal metabolism uniformly in everyone; the response shows clear individual variation.
2. AMPK Pathway
Cell and animal experiments found that PMF can affect:
AMP-activated protein kinase, AMPK
It is an important regulator of cellular energy status.
It subsequently affects a series of metabolic processes, including lipid oxidation and mitochondrial function.
3. PGC-1Ξ± and Mitochondria
In animal/muscle cell experiments, black ginger extract can increase:

as well as indicators related to mitochondrial number and energy metabolism, and improve endurance performance in experimental animals.
However, this cannot be directly translated into:
"Humans will increase mitochondria by consuming it."
Currently, there is not enough human data.
4. Reducing Fat Formation, Increasing Lipolysis
Animal experiments have found that black ginger can affect pathways including:
ACC
FAS
SREBP-1c
CPT1
HSL
such as lipid synthesis, fatty acid oxidation, and lipolysis-related pathways. In high-fat diet mice, decreases in body fat, adipose tissue weight, and adipocyte size were indeed observed.
This falls under:
mechanistic support
and not direct proof of efficacy in humans.
8. A Notable 2025 Human Trial
In 2025, South Korea published a new randomized, double-blind, placebo-controlled trial.
100 people were randomly grouped, and ultimately:
83 people entered the primary analysis
Among them:
40 people in the black ginger group
43 people in the placebo group.
The average BMI was approximately:
27.
One capsule containing SIRTMAX® was taken daily, which included:

This effect is significantly greater than the previous Japanese experiment.
However, this should not be the conclusion:
"50 mg is more effective than 12 mg PMF."
Because it is:
Completely different standardized extracts, different populations, different countries, and different research designs.
Moreover, this study does not provide a direct equivalent PMF dose comparable to "PMF 12 mg."
Meanwhile, in 2025 research:
There was no significant difference in waist circumference between groups, and no clear intergroup improvements were shown in lipid TC, LDL, HDL, or TG.
Therefore, you shouldn't further promote the loss of visceral fat as "improving the entire metabolic syndrome."
9. Less Body Fat Does Not Mean Every Metabolic Marker Improves
This is worth mentioning separately.
A 2018 Japanese study showed improvement in triglycerides.
But in the 2025 South Korean study, even if weight loss, body fat, and visceral fat decrease:
No convincing differences were found between groups in total cholesterol, LDL, HDL, and TG.
Therefore, the safest conclusion at present is:
Black ginger PMF has evidence of "reducing abdominal fat."
Cannot be automatically expanded to:
Lowers blood sugar
Lowers cholesterol
Diabetes prevention
Prevents cardiovascular diseases
Improves fatty liver
These clinical conclusions are still unfounded.
10. Blood sugar aspect: Animal experiments are positive, but human trials have failed to confirm it
This is also a typical "health supplement experiment trap."
In animal experiments, black ginger is often seen:
Blood sugar drops
Insulin improves
HOMA-IR improvement.
For example, experiments with high-fat diet mice have shown such results.
But in 2019, they conducted a special experiment on healthy people:
Black ginger extract:
90 mg/day
Or:
180 mg/day
Continuous:
28 days.
Afterwards, oral glucose tolerance tests are performed.
The conclusion is:
No significant effect of black ginger on human glucose tolerance was found.
So if you see product promotions:
"Black ginger can lower blood sugar"
At present, this cannot be reliably confirmed as a human effect.
11. How Well Are PMFs Absorbed?
The same 2019 study above also conducted human pharmacokinetics.
The researchers focused on three components:
PMF:
3,5,7,3β²,4β²-pentamethoxyflavone
TMF:
5,7,4β²-trimethoxyflavone
And:
5,7-dimethoxyflavone.
The results were quite interesting:
PMF and TMF can be detected in human blood;
However:
No significant blood levels of 5,7-dimethethoflavone were detected under this experimental condition.
Moreover, PMF plasma concentrations increased with 90β180 mg extract doses.
The half-lives measured by PMF and TMF are approximately:
2~3 hours.
So at least we can confirm:
Black ginger PMF does not remain entirely in the intestines; some active methoxyflavonoids can enter the human circulation.
12. How Long Might It Take to Work?
If the goal is:
Reduce abdominal fat
The most convincing trial cycles at present:
12 weeks.
Not three days, not a week.
The 2021 trial was conducted at:
8 weeks
12 weeks
A CT scan was performed, and changes in fat markers were observed at these time points.
So if a person uses:
12 mg PMF/day
It is not advisable to judge its effectiveness solely by weighing the scale within two or three weeks.
A reasonable observation window is approximately:
8~12 weeks.
13. What Dose Has the Best Human Evidence?
If we look only at the most direct human evidence currently available, it can be divided as follows:

Currently, the answer is very clear:

is more valuable as a reference than 'how many mg of black ginger extract.'
14. Would taking more, say 24 mg or 36 mg, be more effective?
There is currently no evidence to prove this.
This is a very important point.
The reason 12 mg is recommended is not because it is the theoretical 'optimal dose,' but because:
it is the dose currently supported by relatively good long-term human RCTs.
There is safety data for higher doses.
A randomized double-blind safety trial of 52 healthy Japanese participants, taking daily:
5 tablets × 150 mg KPFORCE
which is:
750 mg black ginger extract/day
for 4 consecutive weeks, found no adverse events related to the test product, nor any clinical laboratory abnormalities compared to placebo.
A 2021 paper further described this standardized preparation as equivalent to:
PMF 60 mg/day
short-term safety data.
However:
'No issues at 60 mg short-term' β '60 mg is five times more effective than 12 mg.'
There is absolutely no evidence for this.
Therefore, there is no need to intentionally increase 12 mg to tens of mg just to 'boost effectiveness.'
15. What Do We Know About Long-Term Safety?
Currently, human data can support:
12 mg PMF/day × 12 weeks
Overall tolerance is good.
In the 2021 study:
In the placebo group, 10 people experienced 17 adverse events;
In the PMF group, 7 people experienced 9 adverse events.
These were mainly events similar to colds, which the researchers judged to be unrelated to the investigational product. No clinically significant abnormalities were found in blood, biochemical, liver and kidney function, or urine tests.
Another high-dose safety RCT:
750 mg standardized black ginger extract/day × 4 weeks
No safety signals related to black ginger were found either.
The 90-day animal toxicity study also did not find any obvious organ toxicity or genetic toxicity signals.
So, overall:
Short-term safety: relatively reassuring.
However:
Daily use for one year or several years: data is insufficient.
The 2021 paper itself clearly pointed out that longer-term and higher-dose safety studies are needed.
16. A Potential Drug-Interaction Concern
This, on the other hand, rarely appears in product advertisements.
In 2022, Japanese studies specifically researched:
Black ginger and CYP3A drug-metabolizing enzymes.
CYP3A is a very important drug metabolism system in the human body.
In human liver microsome experiments:
Black ginger extract and its components:
5,7-dimethoxyflavone
3,5,7,3β²,4β²-pentamethoxyflavone
can suppress:
CYP3A-mediated midazolam metabolism.
Researchers also observed a significant increase in midazolam exposure in rats after pretreatment with black ginger.
So the correct current understanding should be:
There is a possibility of theoretical and experimental CYP3A drug interactions.
However:
There is no high-quality human drug interaction trial proving that a normal 12 mg PMF dose necessarily causes clinical problems.
If long-term use requires strict control of blood drug concentrations and is mainly metabolized by CYP3A, black ginger should not be added as a regular food but should be checked by a doctor or pharmacist first.
17. What About Male Sexual Function?
Black ginger is often promoted as an aphrodisiac in traditional Thai uses.
There are indeed some mechanisms to support this, such as some PMFs affecting PDE-related pathways and vascular function in experimental systems.
But the human evidence is far less effective than reducing abdominal fat.
A 2018 trial:
14 men aged 50~68;
Self-reported mild erectile dysfunction;
Daily:
100 mg black ginger ethanol extract
Continuous:
30 days.
Thirteen people completed the experiment.
Erectile function, sexual satisfaction, and total scores on the IIEF questionnaire improved.
The question is:
No placebo group.
In other words:
Expectation effects, placebo effects, and natural fluctuations cannot be ruled out.
Therefore, a more reasonable assessment is:
There are signals, but they have not yet been confirmed.
It is absolutely not comparable to the level of evidence for prescription PDE5 drugs.
18. What About Exercise Performance and Fatigue?
The result here is also:
There are some positive results, but they are not stable.
Double-blind RCT with 60 football players:
180 mg black ginger extract/day × for 12 weeks
Right-hand grip strength improves at 4, 8, and 12 weeks;
Left-hand grip strength improved at 8 weeks.
However:
Back and leg strength
40-yard technical testing
Sit and bend your body forward
50-meter sprint
Cardiopulmonary fitness
None of these showed clear intergroup advantages.
So if someone promotes it as:

Boosts explosiveness

The evidence is still far from sufficient.

21. Japan's Foods with Function Claims Are Not Government-Approved for Efficacy
This is especially important.
The Japanese Consumer Affairs Agency clearly states:
Functional food labeling is different from Foods for Specified Health Uses (FOSHU).
Companies only need to submit scientific evidence of safety and functionality before selling to carry functional claims:
The government does not examine the efficacy of each product individually.
The responsibility mainly lies with the applicant company.

The 2025 Korean RCT was also:

funded by a company, with several authors as company employees and another author from SIRTMAX manufacturer Tokiwa.
This does not mean that the research results are necessarily problematic.
RCTs, double-blind, and CT methods themselves remain valuable.
But this means that the current particular need is:
A completely independent team repeated the experiment.
Therefore, overall evidence is more suitable for assessment as "moderate" rather than "very strong."
23. How to Choose a Meaningful Product
This is the most important part when actually buying a product.
If the goal is to achieve the effect of test-based abdominal fat, first check whether the label clearly states:
Polymethoxyflavones derived from Black Ginger: 12 mg/day
Instead of just looking:
Black ginger 500 mg
1000 mg of black ginger concentrate
Black ginger essence is 20 times concentrated
Black ginger enzyme
These numbers themselves are not comparable.
